Manager of Clinical Practice (RN Manager) | Gentiva Health Services | Santa Rosa, CA

As the Manager of Clinical Practice with Gentiva, you will:
- Conduct or delegate the assessment and reassessment of patients,
including updating care plans and interpreting patient needs, while
adhering to Company, physician, and/or health facility procedures and
policies. Manage the assignment of clinical associates.
- Coordinate communication between team members and attending physicians
or clinical associates to ensure the appropriateness of care and outcome
planning. Ensure compliance with third party requirements through review
of documentation and care coordination activities.
- Interpret standards, company policies, and procedures to ensure
compliance with external regulatory authorities and ensure that clinical
associate documentation meets internal standards.
- Consult with third party representatives regarding patient financial
responsibility, service authorization, and reimbursement procedures.
Negotiate reimbursement levels and facilitate the process.
- Participate in the recruitment/interviewing/selection/orientation of
team members, and evaluate their performance relative to job goals and
requirements. Coach staff, recommend in-service education programs, and
ensure adherence to internal policies and standards.
Qualifications
- Associate's Degree or Diploma in Nursing or the equivalent or a
Graduation Degree from an accredited Physical Therapy Program (approved
by the American Physical therapy Association, the Committee on Allied
Health Education and Accreditation of the American Medical Association
and/or the American Physical Therapy Association)
- Meet any additional qualifications as stated in state homecare
regulations
- Minimum of three years experience in area of specialty preferably in a
community or homecare setting
- One year in a supervisory role generally required
- Licensure in the state(s) of practice
- Excellent organization, problem-solving, and communications skills
